Virginia criminal records can be requested through two forms provided by the VA State Police. Both forms can be printed or downloaded from the State Police website. Aside from the State Police, certain VA agencies are also entitled to possess complete criminal record data.

d. Fill out, sign the form, and have it notarized.

2. Criminal Records Search Using SP-230
a. Use this form if you will conduct a criminal history check through any of the following entities.

i. Adult daycare centers
ii. Adult homecare facilities
iii. Cities or counties under an ordinance
iv. Adult district homes
v. Domestic/international adoption/foster-care agencies
vi. Employees of hospital pharmacies
vii. Licensed childcare centers
viii. Licensed adult homes
ix. Unlicensed childcare centers

b. Print or download the form from the VSP forms page (URL below).
i. Fill out the form. There is no need to have it notarized.
ii. Submit the form with a money order/company check for $15 to the Virginia State Police, PO Box 85076, Richmond, VA 23261-5076.
